What insurances do we accept?
We accept most insurances including, but not limited to, Medicare, Medicaid, Blue Cross/Blue Shield, United Healthcare, Aetna and Hometown Health. At this time we are not accepting Humana .

PLEASE NOTE THAT WE ARE NOT ABLE TO ACCEPT MEDICAID AS A SECONDARY INSURANCE UNLESS YOU ARE ON MEDICARE OR HAVE A MEDICARE ADVANTAGE (REPLACEMENT) PROGRAM.

When you arrive for your first appointment, the receptionist will hand you the initial paperwork that we require.

Please arrive 15 minutes prior to your appointment time.
What you need to bring to your first appointment:
Proof of Medical Insurance.
List of prescribed and over the counter medication you use.
Please wear comfortable clothing as you may receive some treatment following your evaluation.
